We offer a comprehensive in-house membership plan that is a non-insurance, affordable option designed to increase access to quality dental care in our community.

The Elevate Membership plan is an option for patients without insurance. It is not insurance. It is non-transferrable, and may only be used in Elevate Dental. 

** For patients who have been diagnosed with periodontal disease, and who need treatment of scaling and root planing (aka ‘deep cleaning’), you will receive a discount of 50% off of treatment – Dental Codes D4341 and D4342. Periodontal Maintenance cleanings – Dental Code D4910 are scheduled thereafter every 3-4 months based on recommendations by your dentist. Your membership credit for Basic Dental cleanings can be applied to Periodontal Maintenance cleanings, and will apply after initial scaling and root planing therapy is completed. Any additional periodontal maintenance cleanings are covered at a discount of 30%.
