We provide all of our new patients with a way to fill out their forms in the comfort of their own home. We are a paperless company. If there are any additional forms when you get in the door, we will have you fill these out on our Ipad. Our process helps you save time. We ask that our patients arrive 10-15 minutes early for their appointment rather than 30-45 on a first time visit.
